[all variants] How do I make a Broadcom Combo Wifi AC + BT 4.0 work in Ubuntu?

Hi,

I have a AzureWave Broadcom BCM94352HMB/BCM94352 802.11/ac/867Mbps WLAN + BT4.0 Half Mini PCI-E Card [14E4:43B1] .
The wifi works fine after installing bcmwl-kernel--source, but the Bluetooth part of the card is not recognized by Ubuntu. I tried upgrading the kernel to Linux 4.4.14-040414-generic but it did not help.

My computer (Toshiba Qosmio X500 - PM55 chipset) has already a bluetooth device (Ver 2.11 + EDR) but its range is extremely limited and I would like to use the capabilities of the combo card.

but the solution did not work for me. Also, I am not sure as to where to get the hex file for this particular card.

Does anyone know how to make it work?
